COVID Waiver:
In consideration of my participation in Geneva Consulting Group’s Event and related events and activities, the undersigned acknowledges and agrees that: I acknowledge the contagious nature of the Coronavirus/COVID-19, and all potential variations and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Geneva Consulting Group has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and all potential variations. I further acknowledge that Geneva Consulting Group cannot guarantee that I will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff of the event’s facility and other event attendees and their families. I am attending voluntarily. I acknowledge that I must comply with all set procedures to reduce the spread while attending the event.
I attest that:
- I will stay home if I am experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I have not traveled internationally within the last 14 days.
- I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
- I am following all CDC-recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
Due to the rise in Delta and Omicron variant cases, at the conclusion of the event, we recommend all attendees follow the guidelines below.
- Get tested 3-5 days after leaving to reduce the potential of community spread.
- Watch for fever (100.4°F), cough, shortness of breath, or other symptoms of COVID-19.
- Should I test positive post-event, I accept the shared responsibility, per CDC guidance, to notify those in close contact of six feet or less for 15 minutes or more.
All Attendees are encouraged to wear a face-covering during the duration of the event.
Geneva Consulting Group strongly discourages guests accompanying attendees.
Geneva Consulting Group agrees to abide by these standards and affirms the same.
I understand and agree that this release discharges Geneva Consulting Group from any and all liability or claim that I, my heirs, or any personal representatives may have against Geneva Consulting with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any activity received from Geneva Consulting Group. This liability waiver and release extends to Geneva Consulting, including its owner, officers, and/or employees, other participants, and if applicable, lessors of premises used to conduct the event.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
